The incumbent is the Staff Physician at a detention facility that houses approximately 500 Immigration and Customs Enforcement (ICE) detainees. The incumbent is responsible to the DIHS Medical Director for all clinical medical, mental, and dental services performed in the outpatient clinic. Responsibilities include planning, implementing, directing, and controlling the clinical aspects of the total comprehensive health care programs. The incumbent is a board certified licensed physician (MD). MAJOR DUTIES As the Staff Physician, the incumbent will provide and oversee care appropriate to patient needs, severity of disease, condition, impairment, or disability within the scope of care provided in the detention setting (short stay unit care vs. ambulatory care) and within the mission, vision, and goals of the organization. Assure that appropriate diagnostic services are available, utilized, and monitored. Serves as on-site Medical Authority within the scope of the services provided by the organization reporting to and seeking guidance from the Medical Director as appropriate. Is responsible for developing and coordinating all aspects of health care delivery at the Service Processing Center. Serves as chairperson of local performance improvement and pharmacy therapeutics. Responsible for appropriate contributions as directed by the Health Service Administrator (HSA) to accreditation preparation and survey process. • Joint Commission on the Accreditation of Healthcare Organizations (JCAHO) • National Commission on Correctional Health Care (NCCHC) • American Correctional Association (ACA) Incumbent has clinical supervision over the Nurse Practioners (NP) and Physician Assistants (PA). In the event that the HSA is a non-clinical person, the incumbent will also have clinical supervisory responsibility over the nursing staff and pharmacy. Serves as a representative of the Division, Bureau, and Agency for clinical matters to national organizations, academic institutions, and other organizations concerned with primary care. Provides program / clinical expertise in the development of site visit protocols and reporting requirements for management of primary care linkages program through SPCs and contract detention facilities. Required to remain abreast of emerging treatment regimens, be conversant clinically with medical literature, provide assistance to national professional associations, and distill medical knowledge into guidance for development of new programs. Assures clinical knowledge, skills, and responsibilities of medical facility and staff. Serves as district expert in the development and delivery of comprehensive primary health care service linkages. Makes presentation to professional, academic, and special interest groups with expert knowledge and serves as an authority on national and international health care trends and a variety of health care service delivery issues. Collaborates with the Medical Director and other senior staff in the development of budget proposals and programmatic plans. Analyzes health care trends of related special population groups and assists in developing and implementing policies and guidelines to improve health services of such groups. Provides leadership and guidance to professional and technical staff in areas of program planning, development, and evaluation. Incumbent will develop and carry out a plan for evaluating program efficiency and effectiveness. Evaluates program goals and objectives, priorities, and activities in terms of current operating experience, program progress, and changing health needs and demands. In this regard, the incumbent develops and monitors an implementation plan in support of overall program goals. At the request of the Director, represents the DIHS to BPHC, Agencies and Offices within the Department of Health and Human Services, Regional Offices, and other Federal and non-Federal agencies, grantees, and advocacy organizations, to ensure coordinated effort in improving health care to the undocumented migrants. Works with representatives of State and local health agencies, as well as professional and community based organizations concerned with improving primary care. Serves as a major focal point for the exchange of information. SUPERVISORY CONTROLS: Incumbent works under the general supervision of the Clinical Director. Supervision is in the nature of national policies and priorities of DIHS and local priorities, which the incumbent has helped to establish for program development and implementation. Coordinates activities with the HSA. (The incumbent must exercise a high degree of independence, initiative, judgment, and follow-through on a wide range of sensitive, complex, and clinical issues. All development and follow-up is performed independently.) [Work is reviewed to determine the successful accomplishment of policy and program objective]. Factor 3 GUIDELINES: 1. Guidelines include agency regulations, the DIHS Policy and Procedure Manual, Joint Commission of Hospital Accreditation Standards, National Commission on Correctional Health Care, and American Correctional Association Standards. While these guidelines are generally applicable, the incumbent must be able to differentiate between the intent and content of the guideline as it pertains to individual situations. 2. As a professional, additional guidelines are expressed in the By-Laws, Rules and Regulations, and the ethics of the medical profession. Factor 4 COMPLEXITY: Incumbent must have a thorough knowledge of standard treatment practices as approved by the American Medical Association. Incumbent must provide primary health care within these standards to patients in a disciplinary setting and be able to apply fundamental management practices with available resources to the delivery of quality health care. The large transient population further increases the complexity of operation by constantly changing the established health care priorities of the detained population, and of individual detainees within the community. The incumbent must have a basic knowledge of health care administration and the capability to assist in planning, developing, and evaluating the health care delivery system of the institution. The medical clinic provides specialties in the following areas: General Practice Orthopedic Surgery* Internal Medicine Oral Surgery* Radiology* Psychiatry General Surgery* Dermatology* Clinical Pathology* Cardiology* OB-GYN* Note: Services (*) are provided by consultants on contract. SCOPE AND EFFECT Incumbent must have a broad understanding of management principles, planning abilities, and treatment modalities. The mental and physical health of the detained population makes the management of the facility particularly difficult due to a changing care load of transient outpatients requiring screening and follow-up during their relatively short stay. The rapid fluctuation of various program priorities and requirement s produces conflicts between program requirements and available resources of personnel, of good medical care and treatment practices, while maintaining appropriate custody requirements. PERSONAL CONTACTS Maintains daily personal contacts with the professional staff, medical staff, institution staff, and other government agencies and reports directly to the Medical Director. PURPOSE OF CONTACTS Contacts are to provide medical care to detained patients on an outpatient program. To provide referral care and placement to community service. To integrate the health delivery system with the overall mission of the DIHS. PHYSICAL DEMANDS General physical well being is an integral part of this position. Good verbal communication skills are necessary, negotiation skills, and some ability to teach. The responsibilities of this position require a great deal of reading and some word processing. Incumbent must be able to perform tasks which may require standing, walking, stair climbing, and negotiating corridors. Many separate and distinct areas with their own physical requirements, i.e., maintenance, pharmacy, psychiatric settings, segregation units, and the health care delivery area. Additionally, the position may be very emotionally stressful. ENVIRONMENT The work is primarily performed in an outpatient health care setting with exposure to potentially violent behavior of detainees, psychiatric patients, and anxious patients. Effective accomplishment of the mission of the DIHS at times requires geographic relocation of supervisory and managerial personnel to meet the legitimate needs of the Division and to better utilize the skills of available staff. Accordingly, the incumbent is subject to said provisions and is susceptible to geographic relocation at the discretion of management in the interest of the efficiency of the service.

Required Skills

Knowledge of the clinical aspects (To serve as a clinical expert in the provision of technical assistance and consultation, and provide for the management of clinically oriented staff). Knowledge of issues confronting the health care delivery system, including problems and concerns of special population groups. Board certification in family practice, internal medicine, or related specialty. License to practice medicine and teaching experience. (To establish a recognized level of expertise in interactions with other clinical providers and for developing and assessing clinical educational programs). Must have a license to practice in the state where the facility is located. Knowledge of congressional mandates, legislative intent, program policies and guidelines, operating procedures, and various departmental initiatives. (To interact with a wide variety of cultures, organizations, and special interest groups). Knowledge of organizations and responsibilities of all levels of government operations. (To communicate with Congress, Federal Agencies, and other related organizations). Mastery of managerial and administrative practices. (To provide expertise in routine management of responsibilities).
